Title: Effect of the Transformative Learning Program on Cultural Competence of Chinese Nurses in a Tertiary Hospital:A pre- and posttest design
Abstract:
Background: The significance of delivering culturally competent nursing care and providing cultural competence training for nurses has grown substantially due to the increasing diversity of patients' cultural backgrounds in China. Presently, however, there is a notable lack of comprehensive cultural competence training programs available for nurses in China.
Objectives: This study aimed to develop and evaluate a transformative learning program on cultural competence of Chinese nurses in a tertiary hospital.
Design: A one group pre- and posttest intervention design.
Settings: A tertiary teaching hospital in China.
Participants: A total of 35 nurses participated in the study.
Methods: A transformative Learning Program on Cultural Competence of nurses was developed using Leininger's Culture Care Theory and Mezirow's transformative learning theory. Cultural competence scores were compared between pre- and posttest using t Wilcoxon Signed-Ranks test.
Results: The mean total score of cultural competence among nurses significantly increased from 111 to 123 (p = 0.001) following participation in the program. Moreover, statistically significant improvements were noted in cultural awareness (from 24 to 26, p = 0.013), cultural knowledge (from 20 to 23, p = 0.034), and cultural understanding (from 14 to 16, p = 0.001).
Conclusions: The study revealed that the transformative learning program designed can enhance cultural competence in China. Characterized by innovative teaching modules, interactive activities, and real - world case studies, this enables nurses to provide patient - centered care respecting diverse cultural backgrounds, improving healthcare quality in a multicultural society. The program is both theoretically sound and practical.

Professor Zhang Lingzhi is a distinguished academic and master’s supervisor. She currently holds multiple prestigious positions, including Vice President of the Zhejiang Nursing Association, Executive Dean of the School of Nursing at Hangzhou Medical College, Provincial Leading Figure in Nursing, Expert of the Provincial Specialized Nurse Training Committee, and Reviewer for projects under the Ministry of Education, Provincial Department of Science and Technology, and Health Commission. Professor Zhang has led nearly 30 research projects funded by the Provincial Department of Science and Technology and the Provincial Department of Education. Additionally, she has served as chief editor or co-editor for 10 provincial-level textbooks and has authored over 40 peer-reviewed publications.
